As part of Guam’s ongoing effort to contain the spread of COVID-19, individuals entering Guam by air or sea shall be subject to a IO-day quarantine, except as provided herein. Travelers arriving into the United States from a foreign country are subject to the Centers for Disease Control and Prevention (CDC) requirement of obtaining a negative pre-departure COVID-19 test result or documentation of recovery from COVID-19, and are required to undergo quarantine based on their traveler status as defined in this document, subject to exemptions provided herein.
This guidance does not apply to individuals identified as close contacts to persons with COVID-19. Close contacts will be subject to quarantine based on applicable Guam law, DPHSS case investigation and quarantine protocols, and CDC guidelines, which may extend the quarantine period beyond the time prescribed herein. Travelers who test positive during their quarantine period or while transiting through Guam are subject to DPHSS isolation protocols and are not subject to this guidance.
Travelers are encouraged to download the Guam COVID Alert application on their mobile device.
I. ARRIVAL PROTOCOLS
Arriving passengers (air or sea) are subject to a IO-day quarantine period at a Government of Guam Quarantine Facility (QFAC), except as provided herein. Government quarantine facilities, and services included therein, are provided at no cost to travelers.
Transiting passengers will remain in a designated section of the A.B. Won Pat International Airport Authority, Guam (GIAA) while awaiting connecting flights less than 13 hours from landing time to departure time, unless the individual qualifies for exemption from quarantine as provided herein.
Transiting passengers awaiting connecting flights in excess of 13 hours will quarantine at the QFAC until they are transported to the GIAA to transit off Guam, unless the individual qualifies for exemption from quarantine as provided herein. Individuals departing by air or sea during their quarantine period will be permitted to leave the QFAC to transit off Guam if they provide verifiable departure information. Transportation will be provided from the QFAC to the GIAA for those traveling by air. However, transportation for individuals departing by sea will be the responsibility of the vessel agent.
All persons exiting the GIAA are required to complete a Health Declaration form prior to or upon arrival into Guam. Individuals subject to Government of Guam quarantine restrictions are requested to voluntarily comply with the Government of Guam quarantine requirements and indicate their willingness by signing a Voluntary Quaramine Acknowledgement form.
By signing a Voluntary Quarantine Acknowledgement form, arriving passengers agree to the following:
1. To comply with the voluntary quarantine order for the period prescribed;
2. To be transported by the Government of Guam directly from the port of entry to the QFAC;
3. To remain in their assigned room while in the designated QFAC;
4. To not receive visitors or visit any other rooms in the facility;
5. To be monitored daily for symptoms of COVID-19 for 10 days after arrival; and
6. To leave the quarantine location only for medical emergencies and medical appointments approved by DPHSS.
Individuals who choose not to sign or do not agree to voluntarily comply with quarantine will be subject to Involuntary Quarantine and issued a Directive for Temporary Quarantine. Individuals who are involuntarily quarantined will also be issued a copy of the 10 GCA Chapter 19 Article 6 and Definitions and notified of their right to legal representation.
Voluntary quarantine is subject to monitoring, verification, and enforcement. The knowing and intentional failure to follow any part of this order constitutes a misdemeanor punishable by a fine of not more than $1,000 or imprisonment of not more than one year or both (10 GCA, Chapter 19, §19604 (c) Cooperation).
Day 5 or Day 6 Test for Release from Quarantine:
Individuals in the QFAC may receive, subject to availability and at no cost to travelers, a COVID-19 viral test on day 5 or day 6 of quarantine, administered by DPHSS or another provider designated by DPHSS.
Currently, there are two types of viral tests accepted by the DPHSS which detect SARS-CoV-2 (COVID-19): molecular tests, such as RT-PCR tests, or isothermal amplification tests, that detect SARS-CoV-2 genetic material, and antigen tests that detect specific proteins on the surface of the virus. These tests identify individuals who are currently infected with SARS-CoV- 2. Test results must include personal identifiers (e.g., patient name and date of birth), specimen collection date, the type of COVID-19 viral test performed, the COVID-19 viral test result, and information about the entity issuing the result (e.g., laboratory or healthcare entity).
If the individual receives a negative COVID-19 viral test result, the individual will be released after completion of day 7 of quarantine and must continue to self-monitor for symptoms of COVID-19, as recommended by DPHSS. If the individual receives a positive COVID-19 viral test result, the individual shall be subject to isolation protocols established by DPHSS and shall be isolated at a Government of Guam Isolation Facility.
In the event the individual declines to receive a COVID-19 viral test while in the QFAC, such individual shall complete the full IO-day quarantine in the QFAC. Upon completion of day 10 quarantine in the QF AC, the individual will be released from quarantine and continue to selfmonitor for symptoms of COVID-19, as recommended by DPHSS.
II. QUARANTINE EXEMPTION PROTOCOLS
Arriving passengers (air or sea) qualifying for an exemption from quarantine under this section are subject to the following requirements:
1. Individuals must observe social distancing, wear protective face coverings, and practice proper handwashing etiquette;
2. Individuals must self-monitor for symptoms of COVID-19; and
3. Individuals must adhere to any Executive Order currently in place for Guam as it pertains to the Public Health Emergency.
A. FULLY VACCINATED INDIVIDUALS:
Individuals who are fully vaccinated with a U.S. Food and Drug Administration (US FDA) Approved, or Emergency Use Authorization (EUA) COVID-19 vaccine, or with a World Health Organization (WHO) Emergency Use Listing (EUL)1 COVID-19 vaccine, may be exempt from quarantine. Travelers having received a combination of accepted US FDA Authorized or EUA COVID-19 vaccine, or WHO EUL COVID-19 vaccines, may be recognized when determining exemption from quarantine. Individuals are considered fully vaccinated after their second dose in a 2-dose series, or after a single-dose vaccine.
Proof of US FDA Approved, EUA, or WHO EUL COVID-19 Vaccination:
Individuals presenting immediate proof of vaccination are eligible to be exempt from quarantine at the port of entry. The submission of false written statements and/or documents with intent to mislead constitutes a misdemeanor (9 GCA Chapter 52).
Individuals seeking an exemption from quarantine under this section must present the following documents upon arrival:
1. Photo identification (e.g., passport, driver’s license, state-issued identification);
2. Primary COVID-19 vaccination record, which includes the patient’s name, date of birth, vaccination date(s), and the brand of COVID-19 vaccine(s) administered; and
3. Secondary form of COVID-19 vaccine verification. Note: Individuals vaccinated in Guam, whose vaccination information are verifiable utilizing the WebIZApplication, will not be required to present a secondary form of vaccine verification.
Acceptable secondary form of COVID-19 vaccine verification includes the following:
1. Vaccinating health authority record; or
2. Vaccinating provider letter; or
3. Receipt of COVID-19 vaccine administered; or
4. Declaration of Individual Attesting to COVID-19 Vaccination, signed under penalty of perjury; or
5. Other form of secondary verification, to be approved at the reviewing officer’s discretion.
Failure to satisfy any of the requirements listed above will result in being subject to Section I, until proof of COVID-19 vaccination is obtained. If proof of COVID-19 vaccination is obtained, the individual will be released from quarantine but must continue to self-monitor.
B. PRE-ARRIVAL NEGATIVE COVID-19 VIRAL TEST
Passengers who present a negative COVID-19 viral test may be exempt from quarantine.
Currently, there are two types of viral tests accepted by the DPHSS which detect SARS-Co V-2 (COVID-19): molecular tests, such as RT-PCR tests, that detect SARS-CoV-2 genetic material, and antigen tests that detect specific proteins on the surface of the virus. These tests identify individuals who are currently infected with SARS-CoV-2. Test results must include patient name, date of birth, specimen collection date, and the type of COVID-19 test performed.
Passengers seeking exemption from quarantine under this section must present the following at the port of entry upon arrival:
1. Photo identification (e.g., passport, driver’s license, state-issued identification); and
2. Valid negative COVID-19 molecular test result collected no more than 3 days prior to departure (boarding a flight to Guam); or
3. Valid negative COVID-19 antigen test result collected no more than 1 day prior to departure (boarding a flight to Guam).
Failure to satisfy any of the requirements listed above will result in being subject to Section I.
The submission of false written statements and/or documents with intent to mislead constitutes a misdemeanor (9 GCA Chapter 52).
C. INDIVIDUALS WITH A PREVIOUS COVID-19 INFECTION
Individuals who have contracted and recovered from COVID-19 within the 3 months preceding their departure to Guam, and who show no symptoms of COVID-19 infection, may be eligible for exemption from quarantine. Individuals seeking an exemption from quarantine under this section must present the following documents upon arrival:
1. Photo identification (e.g. passport, driver’s license, state-issued identification);
2. Valid positive COVID-19 test result collected between 10 to 90 days prior to departure (boarding a flight to Guam); or
3. Valid positive COVID-19 test result collected within 10 days prior to departure (boarding a flight to Guam) and a subsequent clearance from a physician, clinic or health department.
Failure to satisfy any of the requirements listed above will result in being subject to Section I, until proof of previous COVID-19 infection is obtained. If proof of previous COVID-19 infection is obtained, the individual will be released from quarantine but must continue to self-monitor.
The submission of false written statements and/or documents with intent to mislead constitutes a misdemeanor (9 GCA Chapter 52).
III. GOVERNMENT OF GUAM QUARANTINE FACILITY EXEMPTION PROTOCOLS
All passengers who qualify for quarantine at a rental lodging or personal residence under this section must agree to the following:
l. Must sign a Voluntary Quarantine Acknowledgement form;
2. Must provide:
a. Their quarantine location address,
b. A working phone number where they may be contacted, and
c. A working email address;
3. Must not leave the quarantine location for any purpose other than what is outlined in the respective Restriction of Movement (ROM) guidelines provided at the point of entry, for medical emergencies or for medical appointments approved by DPHSS;
4. Must be monitored daily through Sara Alert for symptoms of COVID-19 for 10 days after arrival; and
5. Must return to the Government of Guam Quarantine Facility if found to be non-compliant with symptom monitoring or approved quarantine restrictions.
Individuals quarantined at a rental lodging or personal residence may receive a COVID-19 viral test on day 5 or day 6, subject to availability, administered by DPHSS or another provider designated by DPHSS.
If the individual receives a negative COVID-19 viral test result, the individual will be released after completion of day 7 of quarantine and continue to self-monitor for symptoms of COVID- 19. If the individual receives a positive COVID-19 viral test result, the individual shall be subject to isolation protocols established by DPHSS.
Individuals with departing flights during their quarantine period will be tested and, with a negative result, will be permitted to transit off Guam if they provide verifiable departure information.
A. MINORS
A minor is defined as a person under the age of majority, which legally demarcates childhood from adulthood. The age of majority in Guam is eighteen ( 18). Therefore, persons below the age of 18 are minors, unless they are emancipated minors. Emancipated minors are not subject to this section. An unaccompanied minor is defined as a minor who travels into Guam without the presence of a parent, legal guardian, or authorized representative. Upon arrival into Guam, the airline will follow its protocols for Unaccompanied Minor Service.
In the event parents, legal guardians, or authorized representatives qualifies for exemption from quarantine as outlined in Section II, but the minor does not, the minor will not be required to quarantine upon arrival into Guam.
In the event that the parents, legal guardians, or authorized representatives do not qualify for exemption from quarantine as outlined in Section II, but the minor(s) does, the parents, legal guardians, or authorized representatives may opt to have an authorized adult pick up the minor at the airport, or have the minor stay with them at the quarantine facility. The minor will not be in quarantine. Minors going into the government quarantine facility must remain under the parents, legal guardians, or authorized representatives’ supervision in the assigned room for the duration of their stay. Because the minor was exempt from quarantine at the point of entry, testing is not required for the minor at the facility.
In the event that the parents, legal guardians, or authorized representatives and the minor(s) do not qualify for exemption from quarantine as outlined in Section II, the parents, legal guardians, or authorized representatives may opt to have an authorized adult pick up the minor(s) at the airport where the minor(s) will quarantine at the rental lodging or personal residence of the authorized adult.
An unaccompanied minor arriving in Guam must be met by a parent, legal guardian, or authorized representative at the airport. The parent, legal guardian, or authorized representative must provide photo identification in order to receive the unaccompanied minor. In the event that an unaccompanied minor is not received at the airport by a parent, legal guardian, or authorized representative, DPHSS Child Protective Services will be notified.
B. MEDICAL REQUEST
Medical Request Passenger (MRP): MRPs who do not qualify for exemption from quarantine as outlined in Section II may qualify for quarantine at a rental lodging or personal residence with Restriction of Movement (ROM) guidelines allowing MRPs to leave their quarantine location to attend medical appointments. Such MRPs must meet the following criteria: ( l) the MRP must submit a request to quarantine at a rental lodging or personal residence to DPHSS at dphss.guam.gov/quarantine prior to arrival; (2) such request must contain supporting documentation, including a medical referral from a referring healthcare provider; (3) the MRP must agree to schedule and receive a viral COVID-19 viral test within 24 hours of arrival into Guam.
Individuals with serious or acute medical conditions who do not have a medical referral to receive care on Guam should submit a request at dphss.guam.gov/quarantine prior to arrival.
Emergency medical requests may be considered on a case-by-case basis at the port of entry. The individual will be required to provide supporting documentation for verification of the circumstances.
Care providers and traveling companions will adhere to the same quarantine requirements as the MRPs that they are accompanying. In the event an MRP is subject to quarantine at the designated Government of Guam Facility, such MRP’s care provider and/or traveling companion will likewise be subject to quarantine at the designated Government of Guam Facility, notwithstanding their presentation of qualifications for exemption from quarantine pursuant to Section II.
C. EMERGENCY/HARDSHIP:
Emergency/Hardship (EH): Persons who do not qualify for exemption from quarantine as outlined in Section II may request for EH status due to a personal or family emergency.
Persons seeking EH status must adhere to the following requirements:
1. Must go directly from the port of entry to a Government of Guam Quarantine Facility;
2. Must agree to be assessed by the Medical, Behavioral Health, or Public Health staff at the Government of Guam Quarantine Facility to determine necessary services, which may include an option to leave quarantine on a time-limited basis for a specific EH event/activity or transfer to home quarantine;
a. Individuals must provide documentation and information regarding the emergency/hardship to Medical, Behavioral Health, or Public Health staff;
b. Reviews for emergency/hardship may take several days depending on the completeness of documentation and availability of COVID-19 testing;
3. Must receive a negative COVID-19 viral test result (post-arrival); and,
4. Government of Guam Quarantine Facility Medical, Behavioral Health, and Public Health staff will determine whether a person’s emergency/hardship situation qualifies for an option to leave quarantine for a time-limited activity or transfer to home quarantine on a case-by-case basis.
Persons who are approved to leave quarantine for a specific time-limited EH event/activity. must agree to the following:
1. Return to the Government of Guam Quarantine Facility at the conclusion of the allotted time specified by DPHSS;
2. Wear personal protective equipment as directed by Public Health staff and adhere to social distancing requirements during the entire EH event/activity;
3. Be transported to and from the Government of Guam Quarantine Facility and supervised by authorized Government of Guam staff (e.g., law enforcement officers, National Guard, other authorized DPHSS or Department of Public Works staff, etc.) during the EH event/activity; and
4. Authorized event/activity will typically be approved for two (2) hours in duration.
Failure to satisfy any of the requirements listed above will result in being subject to the original voluntary quarantine order and any accompanying documents.
D. AIRLINE CREW MEMBERS AND FEDERAL AVIATION ADMINISTRATION (FAA) STAFF:
Airline Crew Members who do not qualify for exemption from quarantine as outlined in Section II will be subject to the following guidelines.
Air Crew: A crew member arriving “by air” is defined as flight crew working on the arriving flights or commuting to work on upcoming passenger or cargo flights. Crew members must be listed on the “General Declaration” as provided by the Airline or pre-listed by DPHSS. NonGuam based Air Crew Members must present a valid work identification and their work flight out of Guam upon arrival into Guam. The work flight out of Guam must be within 48 hours of arrival into Guam.
FAA Staff is defined as FAA employees who travel to Guam for the specific purpose of performing official duties in maintaining critical air transportation services. These persons must be non-residents performing official duties in Guam or residents returning from performing official duty and must present a valid FAA work identification upon arrival into Guam.
Resident Airline Crew/FAA Staff must adhere to the following upon arrival into Guam:
1. Must adhere to the general protocols outlined by their employer, designed to mitigate COVID-19 transmission;
2. Must observe social distancing, wear face coverings and practice proper hand-washing etiquette;
3. Are not required to quarantine or undergo COVID-19 testing;
4. Must adhere to any Executive Order currently in place for Guam as it pertains to the Public Health Emergency; and
5. Must self-monitor for symptoms of COVID-19, and immediately notify their primary healthcare provider if symptoms develop.
Non-Resident Airline Crew/FAA Staff must adhere to the following upon arrival into Guam:
1. Must sign a Voluntary Quarantine Acknowledgement form;
2. Must provide:
a. A valid physical address;
b. A working phone number where they can be contacted; and
c. A working email address.
3. Must adhere to the general protocols outlined by their employer, designed to mitigate COVID-19 transmission;
4. Must self-quarantine within their personal residence or rental lodging for 10 days;
5. May only leave the quarantine location for the following purposes: to perform all official duties necessary to maintain critical air transportation services for Guam, purchase groceries, exercise outdoors, pick up curbside/drive-through food services, and for medical emergencies and medical appointments;
6. Must observe social distancing, wear face coverings and practice proper hand-washing etiquette; and
7. Must self-monitor for symptoms of COVID-19, and immediately notify their primary healthcare provider if symptoms develop.
E. VESSEL CREW MEMBERS:
Vessel Crew Members who do not qualify for exemption from quarantine as outlined in Section II will be subject to the following guidelines.
Vessel Crew is defined as crew working on the arriving/departing vessel or commuting to work on an upcoming passenger or cargo ocean vessel. Crew members shall adhere to the general protocols outlined by their Maritime employer designed to mitigate COVID-19 transmission. Vessel crew working on Department of Defense (DoD) vessels are subject to the DoD quarantine guidance. Refer to Section III.F.
Vessel Crew Members Arriving from the GIAA:
Vessel crew members arriving from the GIAA may qualify for quarantine at a rental lodging or personal residence provided the following conditions are met:
1. Crew names must be provided to DPHSS at least 24 hours prior to arrival in Guam by an authorized local vessel agent/employer;
2. A representative of the authorized local vessel agent/employer must be present to provide transportation, or must have made an arrangement with the Department of Public Works to provide transportation of the vessel crew member to the rental lodging or personal residence; and,
3. The vessel crew member must transit to a scheduled vessel or flight departing within 5 days of arrival into Guam.
Vessel Crew Members Arriving from the Seaport:
Vessel crew members arriving at the seaport may qualify for quarantine at a rental lodging or personal residence provided the following conditions are met:
1. Crew names must be provided to DPHSS at least 24 hours prior to arrival in Guam by an authorized local vessel agent/employer;
2. A representative of the authorized local vessel agent/employer must be present to provide transportation or have made arrangement with the Department of Public Works to provide transportation of the vessel crew member to the rental lodging or personal residence;
3. The vessel crew member must transit to a scheduled vessel or flight departing within 5 days of arrival in Guam; or
4. If the vessel crew member is not departing within 5 days of arrival in Guam, the vessel crew member must receive an acceptable negative COVID-19 test result collected on day 5 or day 6 after arrival into Guam. The COVID-19 test may be administered by a provider designated by the local vessel agent/employer.
Vessel Crew Members who do not meet these criteria will be transported by the Government of Guam to the designated Government of Guam Quarantine Facility, where they will quarantine for a period of 10 days.
Vessel Crew Members must adhere to the following requirements:
1. Must sign a Voluntary Quarantine Ack11owledgeme11t form;
2. Must provide:
a. Their quarantine location address;
b. A working phone number where they may be contacted; and,
c. A working email address;
3. Must adhere to the home quarantine Restriction of Movement (ROM) guidelines that will be provided at the point of entry;
4. Must observe social distancing, wear face coverings and practice proper hand-washing etiquette; and
5. Must self-monitor for symptoms of COVID-19, and immediately notify primary care physician if symptoms develop.
F. DOD-AFFILIATED PERSONNEL:
Active-duty military and dependents, DoD civilians and dependents, and DoD contractors arriving by commercial air or sea are subject to the following requirements if they do not meet the criteria outlined in Section II:
1. Active-duty military and dependents, DoD civilians and dependents, and DoD contractors who are on the Joint Task Force listing at the point of entry will follow quarantine guidelines and restriction of movement as established by the DoD.
2. Active-duty military and dependents, DoD civilians and dependents, and DoD contractors who are not on the Joint Task Force listing at the point of entry will be subject to Section I.
3. Active-duty military, DoD civilians, and DoD contractors assigned to visiting units who cannot obtain a negative COVID-19 test due to military mission requirements may still be exempt from quarantine where a cognizant DoD medical officer verifies that such personnel have been embarked at sea or otherwise in a sequestered environment for greater than 14 days and are asymptomatic for the COVID-19 virus.
