ARRIVAL PROTOCOLS:Arriving passengers (air or sea) are subject to a IO-day quarantine period at a Government of Guam Quarantine Facility (QFAC), except as provided herein. Government quarantine facilities, and services included therein, are provided at no cost to travelers.
Transiting passengers will remain in a designated section of the A.B. Won Pat International Airport Authority, Guam (GIAA) while awaiting connecting flights less than 13 hours from landing time to departure time, unless the individual qualifies for exemption from quarantine as provided herein.
Transiting passengers awaiting connecting flights in excess of 13 hours will quarantine at the QFAC until they are transported to the GIAA to transit off Guam, unless the individual qualifies for exemption from quarantine as provided herein. Individuals departing by air or sea during their quarantine period will be permitted to leave the QFAC to transit off Guam if they provide verifiable departure information. Transportation will be provided from the QFAC to the GIAA for those traveling by air. However, transportation for individuals departing by sea will be the responsibility of the vessel agent.
All persons exiting the GIAA are required to complete a Health Declaration form prior to or upon arrival into Guam. Individuals subject to Government of Guam quarantine restrictions are requested to voluntarily comply with the Government of Guam quarantine requirements and indicate their willingness by signing a Voluntary Quaramine Acknowledgement form.[/vc_column_text][/vc_column][/vc_row][vc_row][vc_column][vc_row_inner][vc_column_inner][vc_message]FULLY VACCINATED INDIVIDUALS
Individuals who are fully vaccinated may be exempt from quarantine. Individuals are considered fully vaccinated 2 weeks after their second dose in a 2-dose series, or 2 weeks after a single-dose vaccine.
QUARANTINE EXEMPTION PROTOCOLS
Arriving passengers (air or sea) qualifying for an exemption from quarantine under this section are subject to the following requirements:
1. Individuals must observe social distancing, wear protective face coverings, and practice proper handwashing etiquette;
2. Individuals must self-monitor for symptoms of COVID-19; and
3. Individuals must adhere to any Executive Order currently in place for Guam as it pertains to the Public Health Emergency.
A. FULLY VACCINATED INDIVIDUALS:
Individuals who are fully vaccinated with a U.S. Food and Drug Administration (US FDA) Approved, or Emergency Use Authorization (EUA) COVID-19 vaccine, or with a World Health Organization (WHO) Emergency Use Listing (EUL)1 COVID-19 vaccine, may be exempt from quarantine. Travelers having received a combination of accepted US FDA Authorized or EUA COVID-19 vaccine, or WHO EUL COVID-19 vaccines, may be recognized when determining exemption from quarantine. Individuals are considered fully vaccinated after their second dose in a 2-dose series, or after a single-dose vaccine.
Proof of US FDA Approved, EUA, or WHO EUL COVID-19 Vaccination:
Individuals presenting immediate proof of vaccination are eligible to be exempt from quarantine at the port of entry. The submission of false written statements and/or documents with intent to mislead constitutes a misdemeanor (9 GCA Chapter 52).
Individuals seeking an exemption from quarantine under this section must present the following documents upon arrival:
1. Photo identification (e.g., passport, driver’s license, state-issued identification);
2. Primary COVID-19 vaccination record, which includes the patient’s name, date of birth, vaccination date(s), and the brand of COVID-19 vaccine(s) administered; and
3. Secondary form of COVID-19 vaccine verification. Note: Individuals vaccinated in Guam, whose vaccination information are verifiable utilizing the WebIZApplication, will not be required to present a secondary form of vaccine verification.
Acceptable secondary form of COVID-19 vaccine verification includes the following:
1. Vaccinating health authority record; or
2. Vaccinating provider letter; or
3. Receipt of COVID-19 vaccine administered; or
4. Declaration of Individual Attesting to COVID-19 Vaccination, signed under penalty of perjury; or
5. Other form of secondary verification, to be approved at the reviewing officer’s discretion.
Failure to satisfy any of the requirements listed above will result in being subject to Section I, until proof of COVID-19 vaccination is obtained. If proof of COVID-19 vaccination is obtained, the individual will be released from quarantine but must continue to self-monitor.
B. PRE-ARRIVAL NEGATIVE COVID-19 VIRAL TEST
Passengers who present a negative COVID-19 viral test may be exempt from quarantine.
Currently, there are two types of viral tests accepted by the DPHSS which detect SARS-Co V-2 (COVID-19): molecular tests, such as RT-PCR tests, that detect SARS-CoV-2 genetic material, and antigen tests that detect specific proteins on the surface of the virus. These tests identify individuals who are currently infected with SARS-CoV-2. Test results must include patient name, date of birth, specimen collection date, and the type of COVID-19 test performed.
Passengers seeking exemption from quarantine under this section must present the following at the port of entry upon arrival:
1. Photo identification (e.g., passport, driver’s license, state-issued identification); and
2. Valid negative COVID-19 molecular test result collected no more than 3 days prior to departure (boarding a flight to Guam); or
3. Valid negative COVID-19 antigen test result collected no more than 1 day prior to departure (boarding a flight to Guam).
Failure to satisfy any of the requirements listed above will result in being subject to Section I.
The submission of false written statements and/or documents with intent to mislead constitutes a misdemeanor (9 GCA Chapter 52).[/vc_message][vc_message message_box_color=”warning”]INDIVIDUALS WITH A PREVIOUS COVID-19 INFECTION
Individuals who have contracted and recovered from COVID-19 within the 3 months preceding their departure to Guam, and who show no symptoms of COVID-19 infection, may be eligible for exemption from quarantine. Individuals seeking an exemption from quarantine under this section must present the following documents upon arrival:
1. Photo identification (e.g. passport, driver’s license, state-issued identification);
2. Valid positive COVID-19 test result collected between 10 to 90 days prior to departure (boarding a flight to Guam); or
3. Valid positive COVID-19 test result collected within 10 days prior to departure (boarding a flight to Guam) and a subsequent clearance from a physician, clinic or health department.
Failure to satisfy any of the requirements listed above will result in being subject to Section I, until proof of previous COVID-19 infection is obtained. If proof of previous COVID-19 infection is obtained, the individual will be released from quarantine but must continue to self-monitor.
The submission of false written statements and/or documents with intent to mislead constitutes a misdemeanor (9 GCA Chapter 52).
III. GOVERNMENT OF GUAM QUARANTINE FACILITY EXEMPTION PROTOCOLS
All passengers who qualify for quarantine at a rental lodging or personal residence under this section must agree to the following:
l. Must sign a Voluntary Quarantine Acknowledgement form;
2. Must provide:
a. Their quarantine location address,
b. A working phone number where they may be contacted, and
c. A working email address;
3. Must not leave the quarantine location for any purpose other than what is outlined in the respective Restriction of Movement (ROM) guidelines provided at the point of entry, for medical emergencies or for medical appointments approved by DPHSS;
4. Must be monitored daily through Sara Alert for symptoms of COVID-19 for 10 days after arrival; and
5. Must return to the Government of Guam Quarantine Facility if found to be non-compliant with symptom monitoring or approved quarantine restrictions.
Individuals quarantined at a rental lodging or personal residence may receive a COVID-19 viral test on day 5 or day 6, subject to availability, administered by DPHSS or another provider designated by DPHSS.
If the individual receives a negative COVID-19 viral test result, the individual will be released after completion of day 7 of quarantine and continue to self-monitor for symptoms of COVID- 19. If the individual receives a positive COVID-19 viral test result, the individual shall be subject to isolation protocols established by DPHSS.
Individuals with departing flights during their quarantine period will be tested and, with a negative result, will be permitted to transit off Guam if they provide verifiable departure information.[/vc_message][/vc_column_inner][/vc_row_inner][vc_separator][/vc_column][/vc_row][vc_row][vc_column width=”1/2″][vc_column_text el_id=”Arriving Passengers from Countries with No Reported Cases”]Arriving passengers (air or sea) with direct flight/voyage into Guam that originate from a country with No Reported Cases are not required to have a COVID-19 test and are not required to quarantine. However, such individuals are subject to the following requirements:
- To observe social distancing, wear protective face coverings, and practice proper hand-washing etiquette;
- To self-monitor for COVID-19 symptoms, including the following:
- Loss of smell or taste,
- Fever,
- Cough, runny nose, sore throat,
- Shortness of breath,
- Chills,
- Headache/muscle aches;
- To report signs or symptoms of COVID-19 to their healthcare provider and immediately isolate if they become symptomatic;
- To be tested for COVID-19 at any private or public clinics if they are symptomatic;
- To access beaches or parks only for the purpose of individual exercise and
- To adhere to any Executive Order currently in place for Guam as it pertains to the Public Health Emergency.
- Must sign a Voluntary Quarantine Acknowledgement form;
- Must provide:
- Their quarantine location address;
- A working phone number where they may be contracted;
- The employer name and location where the EHP will be working; and
- A working email address;
- May only leave the quarantine location for the following purposes: to attend work, purchase groceries, exercise individually outdoors, pick up curbside/drive-through food services, and for medical emergencies and approved medical appointments;
- Must adhere to symptom monitoring before each shift and at least four (4) hours after starting each shift by their healthcare organization’s infection control staff;
- Must report any signs or symptoms of COVID-19 to their healthcare organization’s infection control staff, and immediately stop working and isolate if they become symptomatic; and
- May elect to quarantine at the Government of Guam Quarantine Facility alongside spouse and dependents (in the event they are sent to the Government Facility). If electing to quarantine at the Government of Guam Quarantine Facility, EHPs will not be allowed to leave for work.
Individuals departing by air or sea during their quarantine period will be permitted to leave the Government of Guam Quarantine Facility to transit off Guam if they provide verifiable departure information. Transportation will be provided from the Government of Guam Quarantine Facility to A.B. Won Pat Guam International Airport for those traveling by air. However, transportation for individuals departing by sea will be the responsibility of the vessel agent/employer.[/vc_column_text][/vc_column][/vc_row][vc_row][vc_column][vc_separator][/vc_column][/vc_row][vc_row][vc_column width=”1/2″][vc_column_text el_id=”Critical Services Worker (CSW)”]The Department of Public Health and Social Services (DPHSS) issued DPHSS Guidance Memo 2020-11 REV 11 Amendment 3, relative to critical service workers (CSW) arriving into Guam who are not fully vaccinated with a US FDA EUA COVID-19 vaccine. DPHSS Guidance Memo 2020-11 REV 11 Amendment 3 provides additional protocols for CSWs arriving in Guam who are not eligible for exemption from quarantine, as outlined in the Guam Department of Public Health and Social Services (DPHSS) Guidance Memorandum 2020-11 Revision 11, as amended.
CSWs who are fully vaccinated with a US FDA EUA COVID-19 vaccine may qualify for quarantine at an approved rental lodging or personal residence with Restriction of Movement (ROM) guidelines allowing CSWs to report to work during their quarantine period. CSWs are required to present the following at the airport upon arrival:
- an endorsement letter from an authorized DPHSS official and
- an acceptable negative COVID-19 test result collected within 72 hours prior to arrival into Guam.
CSWs seeking ROM but do not present an endorsement letter from an authorized DPHSS official upon arrival shall be subject to the following requirements:
- Must go directly from the port of entry to the Government of Guam Quarantine Facility (QFAC);
- Must request CSW status at the QFAC; and,
- Must agree to receive a COVID-19 test no later than 48 hours.
- In the event an individual receives a positive COVID-19 test result, the individual will be subject to isolation protocols established by DPHSS and will be isolated at a Government of Guam Isolation Facility.
- If the individual receives a negative COVID-19 test result:
- The individual may finish the remaining 10-day quarantine period at an approved rental lodging or personal residence under ROM;
- The CSW may only leave the quarantine location for work and medical emergencies or approved medical appointments;
- Individual must adhere to symptom monitoring by DPHSS; and
- Must report any signs or symptoms of COVID-19 to DPHSS monitoring staff, and immediately stop working and isolate if they become symptomatic
Requests for CSW exemption status must be submitted to the DPHSS website at dphss.guam.gov/quarantine. Requests will be reviewed by an authorized DPHSS official and subject to approval on a case-by-case basis. The CSW will be required to provide supporting documentation for verification of the circumstances.

A minor is defined as a person under the age of majority, which legally demarcates childhood from adulthood. The age of majority in Guam is eighteen (18). Therefore, persons below the age of 18 are minors, unless they are emancipated minors. Emancipated minors are not subject to this section.
Unaccompanied Minors
An unaccompanied minor is defined as a minor who travels into Guam without the presence of a parent, legal guardian, or authorized representative. Upon arrival into Guam, the airline will follow its protocols for Unaccompanied Minor Service.
An unaccompanied minor arriving in Guam without proof of a US FDA authorized COVID-19 vaccination shall complete a IO-day quarantine period at the rental lodging or personal residence identified by the parent, legal guardian, or authorized representative who presents at the airport to assume responsibility of the minor. Such parent, legal guardian, or authorized representative must meet the minor at the airport and provide photo identification in order to receive the minor. In the event that an unaccompanied minor is not received at the airport by a parent, legal guardian, or authorized representative, DPHSS Child Protective Services will be notified.
The parent, legal guardian, or authorized representative will be requested to sign a Voluntary Quarantine Acknowledgement form indicating their willingness to comply with the IO-day quarantine requirements on behalf of the minor. The parent, legal guardian, or authorized representative has the option to have the unaccompanied minor tested for COVID-19 on day 6 after arrival to qualify for an abbreviated quarantine period. DPHSS will contact the parent, legal guardian, or authorized representative to arrange for testing. Negative COVID-19 test results allow the minor to end their quarantine period, subject to symptom monitoring for 14 days after arrival via Sara Alert. Individual is further encouraged to download the Guam COVID Alert App.
Upon DPHSS’s receipt of negative COVID-19 results, the parent, legal guardian, or authorized representative will be notified within 24 to 48 hours that the minor will end quarantine. In the event an individual receives a positive COVID-19 test result, the individual will be subject to isolation protocols established by DPHSS.
By signing a Voluntary Quarantine Acknowledgement form, the arr1vmg minor’s parent, legal guardian or authorized representative agrees to the following:
1. The minor shall comply with the voluntary quarantine order for the prescribed quarantine period;
2. To not receive any visitors at the quarantine location during the quarantine period;
3. The minor shall remain at the quarantine location for the duration of the quarantine period;
4. The minor shall be monitored daily for symptoms of COVID-19 via Sara Alert;
5. The parent, legal guardian, or authorized representative shall report any signs or symptoms observed in the minor to DPHSS quarantine monitoring staff; and
6. The minor shall leave the quarantine facility only for medical emergencies and approved medical appointments.
Unaccompanied minors under this section with departing flights during their quarantine period will be permitted to transit off Guam.
An unaccompanied minor arriving in Guam with proof of US FDA authorized COVID-19 vaccination is not subject to quarantine, however, is subject to symptom monitoring for 14 days after arrival via Sara Alert. Individual is further encouraged to download the Guam COVID Alert App.[/vc_column_text][/vc_column][/vc_row][vc_row][vc_column][vc_separator][/vc_column][/vc_row][vc_row][vc_column width=”1/2″][vc_column_text]AIRLINE CREW MEMBERS AND FEDERAL AVIATION ADMINISTRATION (FAA) STAFF
Airline Crew Members who are not fully vaccinated (Section J.B.) or do not present documentation of a previous COVID-19 infection (Section J.C.) will be subject to the following guidelines.
Airline Crew Members and Federal Aviation Administration Staff are considered critical for the functioning of Guam.
Air Crew: A crew member arriving “by air” is defined as flight crew working on the arriving flights or commuting to work on upcoming passenger or cargo flights. Crew members must be listed on the “General Declaration” as provided by the Airline or pre-listed by DPHSS. NonGuam based Air Crew Members must present a valid work identification and their work flight out of Guam upon arrival into Guam. The work flight out of Guam must be within 48 hours of arrival into Guam.
FAA Staff is defined as FAA employees who travel to Guam for the specific purpose of performing official duties in maintaining critical air transportation services. These persons must be non-residents performing official duties in Guam or residents returning from performing official duty and must present a valid FAA work identification upon arrival into Guam.
Airline Crew Members and FAA Staff must adhere to the following upon arrival in Guam:
1. Must sign a Voluntary Quarantine Acknowledgement form;
2. Must provide:
a. A valid physical address;
b. A working phone number where they can be contacted; and
c. A working email address.
Resident Airline Crew/FAA Staff are further subject to the following requirements:
1.Must adhere to the general protocols outlined by their employer, designed to mitigate COVID-19 transmission;
2. Must observe social distancing, wear face coverings and practice proper hand-washing etiquette; and
3. Are not required to quarantine or undergo COVID-19 testing;
4. Must adhere to any Executive Order currently in place for Guam as it pertains to the Public Health Emergency; and,
5. Must self-monitor for symptoms of COVID-19, and immediately notify their primary healthcare provider if symptoms develop.
Non-Resident Airline Crew/FAA Staff are further subject to the following requirements:
1. Must adhere to the general protocols outlined by their employer, designed to mitigate COVID-19 transmission;
2. Must self-quarantine within their personal residence or rental lodging for 10 days;
3. May only leave the quarantine location for the following purposes: to perform all official duties necessary to maintain critical air transportation services for Guam, purchase groceries, exercise outdoors, pick up curbside/drive-through food services, and for medical emergencies and medical appointments;
4. Must observe social distancing, wear face coverings and practice proper hand-washing etiquette;
5. Must self-monitor for symptoms of COVID-19, and immediately notify their primary healthcare provider if symptoms develop; and
6. Vessel Crew Members who are not fully vaccinated (Section I.B.) or do not present documentation of a previous COVID• 19 infection (Section I.C.) will be subject to the following guidelines.
Vessel Crew is defined as crew working on the arriving/departing vessel or commuting to work on an upcoming passenger or cargo ocean vessel. Crew members shall adhere to the general protocols outlined by their Maritime employer designed to mitigate COVID-19 transmission. Vessel crew working on Department of Defense (DoD) vessels are subject to the DoD quarantine guidance. Refer to Section 11.F.
Vessel Crew Members: To qualify for quarantine at an approved rental lodging or personal residence the following conditions must be met: (1) Crew names must be provided to DPHSS at least 24 hours prior to arrival in Guam by an authorized local vessel agent/employer; (2) a representative of the authorized local vessel agent/employer must be present to provide transportation or have made an arrangement with the Department of Public Works to provide transportation of the vessel crew member to the approved rental lodging or personal residence;
(3) the vessel crew member must transit to a scheduled vessel or flight departing within 72 hours of arrival in Guam; (4) the vessel crew member must present an acceptable negative COVID-19 test result collected within 72 hours of arrival in Guam.
Vessel Crew Members who do not meet these criteria will be transported by the Government of Guam to the designated Government of Guam Quarantine Facility, where they will quarantine for a period of 10 days.
Individuals departing by air or sea during their quarantine period will be permitted to transit off Guam if they provide verifiable departure information. Transportation must be arranged by the vessel agent/employer.
Vessel Crew Members must adhere to the following requirements:
l.Must sign a Voluntary Quarantine Acknowledgement form;
2. Must provide:
a. Their quarantine location address;
b. A working phone number where they may be contacted; and,
c. A working email address;
3. Must adhere to the home quarantine Restriction of Movement guidelines that will be provided at the point of entry;
4. Must observe social distancing, wear face coverings and practice proper hand-washing etiquette; and

Medical Referral Passengers (MRP ): MRPs who are not fully vaccinated (Section J.B.) or do not present documentation of previous COVID-19 infection (Section I.C.) may qualify for quarantine at an approved rental lodging or personal residence with Restriction of Movement (ROM) guidelines allowing MRPs to leave their quarantine location to attend medical appointments. Such MRPs must meet the following criteria: (1) the MRP must submit a request to quarantine at an approved rental lodging or personal residence to DPHSS at dphss.guam.gov/quarantine prior to arrival; (2) such request must contain supporting documentation, including a medical referral from a referring healthcare provider; (3) the MRP must present an acceptable negative COVID-19 test result collected within 72 hours of arrival in Guam. If test results are not provided, the individual must agree to schedule and receive a COVID-19 test within 24 hours of arrival.
Individuals with serious or acute medical conditions who do not have a medical referral to receive care on Guam should submit a request at dphss.guam.gov/quarantine prior to arrival.
Emergency medical requests may be considered on a case-by-case basis at the port of entry. The individual will be required to provide supporting documentation for verification of the circumstances.
MRPs must adhere to the following requirements:
1. Must sign a Voluntary Quarantine Acknowledgement form;
2. Must provide:
a. Their quarantine location address,
b. A working phone number where they may be contacted, and
c. A working email address;
3. Must adhere to MRP ROM guidelines that will be provided at the airport;
4. Must adhere to any Executive Order currently in place for Guam as it pertains to the Public Health Emergency; and,
5. Must report any signs or symptoms of COVID-19 to DPHSS quarantine monitoring staff.
Individuals with departing flights during their quarantine period will be permitted to transit off Guam if they provide verifiable departure information.
To qualify for quarantine at an approved rental lodging or personal residence with applicable Restriction of Movement, care providers and traveling companions of MRPs must present an acceptable negative COVID-19 test result collected within 72 hours of arrival in Guam. If test results are not provided, the individual must agree to schedule and receive a COVID-19 test within 24 hours of arrival. Care providers and traveling companions will adhere to the same quarantine requirements as the MRPs that they are accompanying. In the event an MRP is subject to quarantine at the designated Government of Guam Facility, such MRP’s care provider and/or traveling companion will likewise be subject to quarantine at the designated Government of Guam Facility, notwithstanding their presentation of an acceptable negative COVID-19 test result. Emergency/Hardship: Persons who do not qualify for exemption from quarantine as outlined in Section I.B. or Section I.C. may request for EH status due to a personal or family emergency.
Persons seeking EH status must adhere to the following requirements:
1. Must go directly from the airport to a Government of Guam Quarantine Facility;
2. Must agree to be assessed by the Medical, Behavioral Health, or Public Health staff at the Government of Guam Quarantine Facility to determine necessary services, which may include an option to leave quarantine on a time-limited basis for a specific EH event/activity or transfer to home quarantine;
a. Individuals must provide documentation and information regarding the emergency/hardship to Medical, Behavioral Health, or Public Health staff;
b. Reviews for emergency/hardship may ta1ce several days depending on the completeness of documentation and availability of COVID-19 testing;
3. Must receive a negative COVID-19 test result {post-arrival); and,
4. Government of Guam Quarantine Facility Medical, Behavioral Health, and Public Health staff will determine whether a person’s emergency/hardship situation qualifies for an option to leave quarantine for a time-limited activity or transfer to home quarantine on a case-by-case basis.
Failure to satisfy any of the requirements listed above will result in being subject to the original voluntary quarantine order and any accompanying documents.
Persons. who are approved to leave quarantine for a specific time-limited EH event/activity, must agree to the following:
1. Return to the Government of Guam Quarantine Facility at the conclusion of the allotted time specified by DPHSS;
2. Wear personal protective equipment as directed by Public Health staff and adhere to social distancing requirements during the entire EH event/activity;
3. Be transported to and from the Government of Guam Quarantine Facility and supervised by authorized Government of Guam staff (e.g., law enforcement officers, National Guard, other authorized DPHSS or Department of Public Works staff, etc.) during the EH event/activity; and
4. Authorized event/activity will typically be approved for two (2) hours in duration.
Persons who are approved to quarantine at approved rental lodging or personal residence must agree to the following:
1. Must sign a new Voluntary Quarantine Acknowledgement form;
2. Must provide:
a. Their quarantine location address,
b. A working phone number where they may be contacted, and
c. A working email address;
3. Must not leave the quarantine location for any purpose, except to attend to the emergency/hardship at authorized times;
4. Must adhere to symptom monitoring by DPHSS; and
5. Must return to the Government of Guam quarantine facility if found to be non-adherent to symptom monitoring and/or quarantine.
Individuals departing by air or sea during their quarantine period will be permitted to leave the Government of Guam Quarantine Facility to transit off Guam if they provide verifiable departure information. Transportation will be provided from the Government of Guam Quarantine Facility to A.B. During this 14-day period, take these steps to monitor your health and practice social distancing:
- Take your temperature with a thermometer two times a day and monitor for fever. Also watch for cough or trouble breathing. Use this temperature log to monitor your temperature.
- Stay home and avoid contact with others. Do not go to work or school.
- Do not take public transportation, taxis, or ride-shares.
- Keep your distance from others (about 6 feet or 2 meters).
There is widespread, ongoing transmission of novel coronavirus worldwide (see Global COVID-19 Pandemic Notice). If you have traveled internationally in the past 14 days, stay home and monitor your health.
Effective after midnight on Wednesday July 1, 2020, the following changes will be implemented:
- Hotspot locations will be renamed as “High Risk Areas”
- For the US, risk status will be determined on a state-by-state level, to include territories.
- The default level of quarantine required upon arrival on Guam is based on whether or not a traveler is coming from a High Risk Area.
- A Pre-Arrival Negative Polymerase Chain Reaction (PCR) Test, in particular those traveling from a High Risk Area, is necessary to determine the location of quarantine required and eligibility to qualify for an abbreviated quarantine length.
- Residency status will NOT determine the level of quarantine or testing required. Considerations for alternate quarantine locations will still be made in certain limited circumstances.
- High Risk locations will be determined by a calculated COVID-19 Area Risk (CAR) score, which primarily assesses an area’s recently calculated Case Doubling Time and Test Positivity Rate.
Current high risk areas include: Alabama, Arizona, Arkansas, California, Florida, Georgia, Idaho, Mississippi, Nevada, North Carolina, Oklahoma, Oregon, South Carolina, Tennessee, Texas, Utah, Bangladesh, India, Indonesia, Nepal, Philippines, Africa, Central America, Middle East, and South America.

The purpose of this Guidance is to provide further clarification on the voluntary quarantine requirements for all persons arriving in Guam.
The following clarification is provided and is applicable to Executive Order 2020-04, 2020-11, 2020-14, 2020-16, and 2020-20 and any other Executive Orders regarding entry into Guam and the March 31, 2020 Public Health Directive relative to quarantine of all persons arriving in Guam.
As part of Guam’s effort to contain the introduction and spread of COVID-19, all persons entering Guam by air or sea from international hot spots or from international non-hot spots and domestic locations shall be subject to quarantine locations for a period of 14 days:
Arrivals from International Hot Spots
1. ALL persons having originated from or traveled through international locations deemed to be “hot spots” shall be subject to 14-day quarantine at a GovGuam qualified quarantine facility. * See definition
2. Considerations for alternate quarantine locations will be made in certain limited circumstances, where medical or other particular concerns are implicated.
3. Obtaining a negative test result is NOT a requirement for hot spot travelers, nor will it alter the quarantine requirements.
Arrivals from International Non-Hot Spots and Domestic Locations
1. ALL persons traveling domestically or having originated from or traveled through international NOT deemed a “hot spot” shall be subject to 14-day quarantine at a GovGuam qualified quarantine facility.
2. Guam Residents shall be allowed to complete their 14-day quarantine at their home residence.
3. Guam Non-Residents can present a valid diagnostic laboratory COVID-19 PCR negative test result for SARS CoV2, that is not more than one (1) week from the date of attempted entry into Guam, and shall complete the 14-day quarantine requirement at their personal residence or rental lodging.
* A “Hot Spot” is defined as a country/location with COVID-19 case doubling rate less than every 30 days. A “Non-Hot Spot” location has a COVID-19 case doubling rate greater than or equal to every 30 days. Countries/locations hot spot status will ultimately be determined and updated by the Department of Public Health and Social Services in consultation with the Governor and State Surgeon’s Physician Advisory Group. All non-residents are subject to 14-day quarantine at a GovGuam designated quarantine facility. Any and all costs associated with the quarantine and/or treatment of individuals who are subject to restricted entry into Guam pursuant Executive Orders 2020-04, 2020-11, 2020-14, and 2020-16, shall be the responsibility of the individual and the carrier that the individual contracted with to travel to Guam.
In coordination with the Guam International Airport Authority, transiting passengers will be asked to remain in designated portions of the airport facility while awaiting connecting flights less than 10 hours.
Compliance with voluntary quarantine regardless of location is subject to monitoring, verification, and enforcement. The knowing and intentional failure to follow any part of this order constitutes a misdemeanor punishable by a fine of not more than $1,000 or imprisonment of not more than one year or both (10 GCA, Chapter 19, §19604 (c) Cooperation).

1. Who is considered a “non-resident”?
A non-resident is considered as a person who does not live and work on Guam.
2. When does the mandatory quarantine take effect?
The mandatory quarantine will take effect immediately 12:01AM Thursday, March 19, 2020.
3. What happens to passengers of flights that arrived before the mandatory quarantine took effect?
The travel restrictions were implemented on March 16, 2020, but not enforced. However, if they are feeling ill and displaying symptoms they are to contact the Department of Public Health & Social Services at 671-734-7143 or 671-888-9276.
4. The guidelines state that the mandatory quarantine will apply unless the traveler possesses a Department of Public Health & Social Services recognized or certified document (certificate) that shows the traveler tested negative for COVID-19. What is considered a recognized and certified document (certificate)?
Several countries are requiring certified documentation similar to Guam’s local mandate as a condition of entry. The certificate must include results from a lab accredited by the World Health Organization (WHO) or the Centers for Disease Control and Prevention (CDC) that an individual tested negative for COVID-19. While DPHSS is aware that countries are developing a uniform certificate to satisfy the legal requirements, NO CERTIFICATE HAS BEEN PRE-APPROVED BY DPHSS at this time. Additionally, the certificate must be in English.
5. How many protocols are there?
There are currently (2) protocols. One for Manila travelers and one for Non-Manila travelers. Please refer to the supplemental documents attached to this correspondence from the Government of Guam’s Joint Information Center.
6. If a non-resident traveler does not have any health clearance certification that is recognized by DPHSS, will they be mandatory quarantined?
All non-resident travelers are subject to quarantine.
7. What will happen if a non-resident does not volunteer to be quarantined?
The quarantine will be enforced and they will be transported to an approved quarantine site.
8. Which hotel is accepting non-resident travelers?
GHRA is coordinating with the hoteliers to self-identify who will be accepting asymptomatic nonresident travelers during this quarantine period. Should a asymptomatic non-resident traveler be denied access, the government will coordinate relocation to a participating hotel.
9. What if I decline asymptomatic travelers at my hotel?
You must notify DPHSS and the asymptomatic non-resident will be transported to an approved quarantine site.
10. If I am a non-resident traveler, how will I know if my pre-booked hotel is an approved quarantine site?
You will be informed at the airport by government officials. If your pre-booked hotel is a participant you will be transported to the facility by the government. If your pre-booked hotel is not a participant you will be escorted to an alternate facility.
11. How do you ensure proper quarantine is enforced at the hotels?
As part of their pandemic plan, hotels should have identified certain floors to be used exclusively for quarantine. The hotels and the Government of Guam will work together to ensure safe practices while servicing guests.
12. What if I am not staying in a hotel but a vacation rental?
You will be transported to the approved quarantine facility.
13. Who will cover my expenses during this quarantine period?
The non-resident traveler and airline carrier will cover the expenses.
14. Who is handling the ground transportation from the airport to the quarantine facility for the nonresident travelers?
The Government of Guam will enforce and provide transportation.
15. The quarantine period is for (14) days. Will a non-resident traveler be allowed to return home sooner?
If they are asymptomatic they will be allowed to fly home sooner than the (14) day period and will be transported from the quarantine facility to the airport by the government. If they plan to stay longer than (14) days they will be released from quarantine after the 14th day, granted that they remain asymptomatic.
16. What happens during the quarantine period?
Asymptomatic non-residents are confined to their room and subject to public health screenings at least twice a day. Any in-room service will be coordinated between facility staff and the government to ensure minimal exposure.
17. What if I am just a transiting passenger?
Transit passengers will go through regular airport protocols and allowed to pass through Guam. If they are planning to stay overnight, they will be subject to quarantine.
18. Do these protocols apply to flight crew?
If the flight crew is transiting on a turn around flight they are exempt but still subject to health screenings. If the flight crew is transiting overnight there are some requirements. If they are asymptomatic, crew will be required to undergo in-room quarantine in a pre-approved hotel. If they are symptomatic, crew will be required to undergo in-room quarantine for the 14 day period in a pre-approved hotel secured by the government.
19. Do these protocols apply to residents?
If the returning resident is arriving from Manila, Philippines they will have to undergo an involuntary quarantine at a designated facility. All other residents arriving from other destinations will be asked to self-quarantine for 14 days at home. If DPHSS does not find the home suitable to
self-quarantine, they will be transported to an approved facility.
20. Why are these protocols being implemented across the board for all travelers when Japan and Korea have demonstrated proper containment?
As part of the community mitigation strategy, the Government of Guam is trying to stop the inflow of infected carriers and slow down transmission in the community.
21. How do we ensure proper sanitation measures are taking place at a quarantine facility?
The approved quarantine facility has implemented pandemic plans to ensure protocols are in place for regular sanitation and provide protection for employees and guests.
22. Are quarantine facilities decontaminated at the end of the quarantine period?
Yes, protocols and decontamination procedures are implemented. The Government of Guam is working with quarantine facilities to ensure decontamination takes place for up to 15 days. The government will also ensure decontamination methods are EPA and OSHA compliant.
23. How are tourism industry employees protected while working at quarantine sites?
All approved quarantine sites are reviewed and approved by the Guam Police Department, Guam Fire Department and DPHSS before receiving asymptomatic non-resident and resident guests. The Government of Guam also coordinates with the facilities to follow the pandemic plan for employees to operate back of the house functions to ensure they are protected and not have direct contact with quarantined individuals.The government has provided staffing to operate on quarantined floors to further protect industry employees from exposure and ensure enforcement of quarantine protocols.
24. Who stays at a quarantine facility?
Only asymptomatic residents who arrived from Manila, Philippines and non-resident guests stay at a quarantine facility. They are subject to regular temperature checks and other screening methods before being released from the quarantine period by DPHSS.
25. If a guest becomes symptomatic, do they stay at a quarantine facility?
No. An asymptomatic guest that becomes symptomatic will be transferred to isolation at anapproved medical facility.
26. If I am coming from a destination that does not have any COVID-19 cases, am I exempt from mandatory quarantine?
