Essential Healthcare Professionals with DPHSS Endorsement Letter

To qualify for quarantine at an approved rental lodging or personal residence with Restriction of Movement (ROM) guidelines allowing EHPs to report to work during their quarantine period, EHPs are required to present the following at the airport upon arrival:
(1) an endorsement letter from the DPHSS Director and,
(2) an acceptable COVID-19 negative test result collected within 5 days of arrival in Guam. EHPs arriving without an endorsement letter from the DPHSS Director and an acceptable COVID-19 negative test result will be transported by the Government of Guam to the designated Government of Guam Quarantine Facility, where they will remain until they are able to obtain both, or be subject to quarantine for a period of 14 days. DPHSS will facilitate testing at the site as necessary, subject to availability.
In order to qualify as an EHP under this section, persons seeking EHP status based on affiliation with Department of Defense (DoD) operations must obtain an exception to applicable DoD quarantine policies in coordination with cognizant military authorities. (See Section J)
To qualify for quarantine at an approved rental lodging or personal residence, accompanying spouses and dependents of EHPs must arrive on the same flight with such EHPs and present an acceptable COVID-19 negative test result collected within 5 days of arrival in Guam. Accompanying spouses and dependents must adhere to quarantine requirements at their approved rental lodging or personal residence. Accompanying spouses and dependents arriving without an acceptable COVID-19 negative test result will be transported by the Government of Guam to the Government of Guam Quarantine Facility, where they will remain until they receive a negative test result, subject to availability, or be subject to quarantine for a period of14 days. In the event an EHP is subject to quarantine at the designated Government of Guam Facility, such EHP’s accompanying spouse and dependents will likewise be subject to quarantine at the designated Government of Guam Facility, notwithstanding their presentation of an acceptable COVID-19 negative test result.
In the event the EHP is traveling with dependents who are minors and without a spouse, the minor will be allowed to quarantine alongside the EHP. DPHSS will facilitate testing for minors arriving in Guam without a COVID-19 negative test result.

Currently there are two types of diagnostic tests accepted by the DPHSS that detect SARS-CoV-2 (COVID-19): molecular test, such as RT-PCR tests, that detect the virus’s genetic material, and antigen tests that detect specific proteins on the surface of the virus. These tests help to identify whether individuals are currently infected with the coronavirus. In each case, the individual’s specimen must have been collected within 5 days of arrival in Guam. At a minimum, test results must include patient name, date of birth, specimen collection date, and the type of test run.
Essential Healthcare Professionals must adhere to the following requirements:
- Must sign a Voluntary Quarantine Acknowledgement form;
- Must provide:
- Their quarantine location address;
- A working phone number where they may be contracted;
- The employer name and location where the EHP will be working; and
- A working email address;
- May only leave the quarantine location for the following purposes: to attend work, purchase groceries, exercise individually outdoors, pick up curbside/drive-through food services, and for medical emergencies and approved medical appointments;
- Must adhere to symptom monitoring before each shift and at least four (4) hours after starting each shift by their healthcare organization’s infection control staff;
- Must report any signs or symptoms of COVID-19 to their healthcare organization’s infection control staff, and immediately stop working and isolate if they become symptomatic; and
- May elect to quarantine at the Government of Guam Quarantine Facility alongside spouse and dependents (in the event they are sent to the Government Facility). If electing to quarantine at the Government of Guam Quarantine Facility, EHPs will not be allowed to leave for work.
Individuals departing by air or sea during their quarantine period will be permitted to leave the Government of Guam Quarantine Facility to transit off Guam if they provide verifiable departure information. Transportation will be provided from the Government of Guam Quarantine Facility to A.B. Won Pat Guam International Airport for those traveling by air. However, transportation for individuals departing by sea will be the responsibility of the vessel agent/employer.[/vc_column_text][/vc_column][/vc_row]
